Levels of Care

Respite Care

Respite Care provides the caregiver/family members an opportunity to take some time for themselves while ensuring their loved one is cared for.  Respite Care is available on any of our units for whatever length of time is needed.


Skilled Nursing / Assisted Living Care



Assisted Living - Simple directions and reminders are provided to residents in the Assisted Living Unit by our staff – and group activities are made available for residents to take part in, as they please.

Skilled Care - Along with simple directions and reminders, our staff provides higher levels of supervision to Skilled Care residents.  Both group and one-on-one activities are made available for residents to participate in as they please.



Assisted Living - For the times it may be needed, minimal assistance is provided by our staff to help Assisted Living residents with their mobility. If a wheelchair is used, residents must be able to transfer themselves without the assistance of staff.

Skilled Care - More assistance with mobility is available to residents in Skilled Care. Staff assistance is provided to transfer residents from one location to another (i.e. bed to wheelchair) or when walking. As they are needed, mechanical lifting devices are used to ensure safe transfer of residents.


Dressing & Bathing

Our staff provides Assisted Living residents minimal assistance with dressing (i.e. with zippers, support hose, tying shoes). To ensure safety, the staff is close by to assist with showering or tub bathing – to the extent that residents request such assistance.

Complete bathing or showering is provided by our staff – as it is requested by Skilled Care residents or necessitated by the residents’ inability to do so on their own. Additionally, the staff provides complete dressing and clothing changes, as they are necessary.



Mealtime is a favored time by all residents of Meadowbrook. Our restaurant style dining allows residents the choice of several entrée choices for every meal. Our meal program ensures residents’ therapeutic diet needs are met, as well as any individualized nourishment requirements.

The dietary needs of residents in Skilled Care are monitored carefully to ensure both nutrition and safety. As is the case throughout Meadowbrook, a registered dietician oversees the meal program. Staff is available to assist residents and to attend to the individual meal preferences of each resident. If necessitated, the staff will provide the care associated with a resident’s gastric tube or intravenous feedings.


Personal Care

Our staff provides support to promote independence and cleanliness for residents in Assisted Living when using the restroom. Stand-by assistance and/or verbal cuing can also be provided by the staff, as it may be needed.

Our staff provides Skilled Care residents with full assistance with their toileting needs. Additionally, the staff can provide catheter care, as ordered by the resident’s physician.


Treatment & Nursing Care

The daily care of Assisted Living residents is coordinated by a licensed nurse and is provided by certified nurse assistants. To the extent this care may be needed, we provide services such as routine lab and physical assessments and ensuring residents take their medications as ordered by their physician.

Because of the higher level of care required by Skilled Care residents, a licensed nurse is on duty at all times to coordinate care, while certified nurse assistants provide residents their daily care. The resident’s physician is responsible for prescribing treatments, medication and the level of ongoing observation. Nursing staff provide ongoing care, such as intermittent or continuous oxygen therapy; intravenous therapy; and sterile procedures.

We understand the issues that residents of long-term care facilities sometimes face.
